Testing Policies and Information

    Teacher Selection

  1. Only current Florida certified teachers are used to administer tests during
    group testing.
  2. Teachers are carefully selected by H.E.R.I. based upon questionnaire results,
    interviews and recommendations.
  3. Test Booklets and Test Results

  4. Test booklets and materials are available to only the Testing Coordinator and
teachers. All testing materials and copies of test results are kept secured with
only the Testing Coordinator and H.E.R.I. Board members having access.
  5. Test booklets will not be available for parents to view on site at testing.
  6. H.E.R.I. has reviewed the testing percentile ranking for Duval County. It will
now be H.E.R.I.’s policy that a student must have an NPR ranking of 25% in
order for H.E.R.I. teachers to sign a Home Education Evaluation Form.
  7. If test results are used to compile statistics or other studies, only copies
without names will be provided to H.E.R.I. Board members.
  8. Copies of test results will only be given to the parent or legal guardian who
registered the child. Only one copy of test results will be provided to the
parent/legal guardian who registered the child. If an additional copy is
requested at any time, there will be a $15 fee, paid to HERI. Additional copies
for the same child or for siblings will incur a fee of $10 each. These copies
will only be provided to the parent/legal guardian who originally registered
    the child for testing.
  9. H.E.R.I. DOES NOT submit test results to county superintendents’ offices.
They are only provided to the parent or legal guardian who registered them.
    H.E.R.I.’s Testing Policy:
  10. If any issues arise during testing, H.E.R.I. reserves the right to ask parents to
make other arrangements for a child’s annual educational evaluation upon
recommendation by the teacher administering the test and/ or Testing
    Coordinator’s recommendation.
  11. H.E.R.I. reserves the right not to retest a child in the future if parents have
been notified of a discipline problem or unresolved problem during a group
    testing event.
  12. Children must be the age of five as of September 1st, regardless of grade
placement, in order to test with H.E.R.I.
  13. Student Policies

    Test Booklets

  14. There is a $10 fee for any student who writes, doodles, or draws in a test
    booklet.
  15. Scratch paper and pencils will be provided to all students. Please encourage
your child to fill in the test bubbles completely and clearly. Students should
press firmly enough to make a dark bubble. If they fail to do so, this could
    affect test results.
  16. Access

  17. All students must stay inside testing facility during testing days and be
supervised by teachers, aides, Testing Coordinator or parents at all times.
  18. The full time allotted for sub-tests will be recognized and ALL STUDENTS
MUST stay in the classroom and wait quietly even if they finish early. This
includes high school students as well.
  19. Students may not be in unoccupied classrooms at any time. They may only
be in their designated classrooms or in designated waiting areas with a teacher
    or aide.
  20. Students who drive themselves to the testing location will need to check in
and check out each day. This can be modified if H.E.R.I. is notified prior to
    testing.
  21. Disturbance

  22. Any child who poses a disturbance (any physical or verbal attack) or discipline
problem (any behavior which the teacher feels is inappropriate) will not be
allowed to complete testing. The parent will be notified to pick the child up
immediately. Refunds WILL NOT be issued.
  23. Electronics

  24. All cell phones, smart watches, smart glasses, earbuds and any other
electronic devices must be turned off while in the classroom and placed under
the student’s chair during the entire time they are in the testing room. Please
make sure that your child is aware of this policy. Please also let them know
that if they finish a section of their test earlier than the allotted time, they may
NOT use their cell phones or other electronic devices while they wait for
others to finish that section of the test. If a student’s cell phone rings, beeps
or the student is found holding or using their cell phone or other electronic
devices, they will be asked to leave the classroom and forfeit the remainder of
the sub-test. The Testing Coordinator will notify the parent and will determine
if the student will continue testing. HERI is not responsible for lost, stolen or
damaged items that are brought to the testing site.
  25. Illness

  26. Children who become ill and may be contagious on the day of testing should
not come to the testing site. Parents should contact the testing coordinator
    immediately using the email: herijax@yahoo.com
  27. Children who have contagious illnesses (fever, virus, chicken pox, etc.) are
    not eligible for H.E.R.I. testing.
  28. Cheating

  29. Any child found cheating will not be allowed to complete testing and the
parent will be notified to pick the child up immediately. Refunds WILL NOT
    be issued.
Cheating during the testing period is defined:
    a) Providing answers to, or receiving answers from any other person or
    source
not previously authorized regardless of its physical
location. Examples include, but are not limited to looking at others’
answer sheets or consumable test booklets.
    b) Use of calculators or any other electronic devices including cell phones for
    ANY purpose.
    c) Changing answers after timed sessions or previously administered test
    d) Working ahead on test not yet administered
    e) Continuing to work on a test after time has been called
    f) Providing answers to another student
    g) Removing test materials from classroom

  31. Conservative dress will be required for all students while testing with
    H.E.R.I.
    a. Conservative dress will be required for all students while testing with
H.E.R.I. Please be sure that your children are appropriately dressed

sayings and pictures. Clothing should not be tight or revealing and no
    undergarments should be visible.
    b. Students who arrive at H.E.R.I. testing dressed inappropriately (in the
judgment of the classroom teacher and/or Testing Coordinator) will be
removed from testing. The student will need to go home and change.
They will miss the testing sub-tests given during that timeframe.
(Please see policy below (#25) on late arrivals and missed tests.)
    c. If a student misses any portion of their test due to inappropriate dress
the parent will be responsible for contracting with a teacher to have the
test completed. Additional fees will be required.
  32. Late Arrivals

  33. Late arrivals will not be allowed to enter the testing room while testing is in
progress. Any student who arrives late to testing will be asked to wait for the
next sub-test to begin. No individual tests or make-up tests will be available
for those who are late or absent. Parents will need to reschedule with the
teacher and pay an hourly fee for all make-ups. It will be the responsibility of
the parent to work with the Testing Coordinator before the original testing day
is finished. All make-ups must be completed during the two day testing time
frame and at the same testing location. Additional fees will be charged; the
fee is $20 per hour, paid directly to the teacher after testing is complete
  34. Dismissal

  35. Your student will be dismissed from the gym/fellowship area to the
parent/guardian we have on record based on our registration forms.
  36. The finish times are estimates. Our teachers will be focused on administering
the test in a time frame that is conducive to the best situation for the students.
  37. Food and Smoking

  38. No snacks, drinks or gum will be allowed in the classroom.

    policies and information outlined herein.
